Connect: Remote Therapeutic Monitoring (RTM)
Our RTM program uses periodic text message check-ins to gather information about how your treatment is progressing. Your responses help us fine-tune your care in real time.
You’ll receive text-based questions covering:
- Pain score assessments
- Feedback on medication and procedures
- Insights into your personal pain management strategies
By replying to these messages, you help us monitor your progress and make any needed adjustments to ensure you’re receiving the most effective treatment.
Connect: Principal Care Management (PCM)
The PCM model involves monthly phone check-ins with our clinical care team. One of our compassionate nurses will call to review patterns in your pain and evaluate how well your current treatment plan is working.
This option focuses on phone-based communication, though texting is available if you need to reach out to our team between calls. You’ll be responsible for initiating those texts for urgent or timely concerns.
